The CrossCare project (funded by the Italy-Slovenia programme) will celebrate de European Cooperation Day this year.
The common territorial challenges faced by the CrossCare project are related to the phenomenon of active aging with its important economic and social implications. In particular, the project action focuses on:
- Responding to common challenges with a cross-border memorandum of understanding that will contribute to strengthening institutional and administrative capacity in the implementation of social and health services which, through the promotion of active aging, can be an element of social and competitive cohesion in economic terms.
- Development of a cross-border model for the timely multi-professional care of the elderly and his family through the adoption and testing of an integrated model of accompaniment along the path of seniority, based on a welfare community methodology.
- Establishment of Senior Services Points within the Retirement Homes (CdR), open to the territory and innovative as regards the implementation of pilot actions aimed at encouraging the taking charge of the elderly and his family in a personalized manner, global and continuous, at regional and cross-border level.
